Job Summary: A Registered Dietitian creates nutrition plans to improve the health and physical conditions of residents. The main duties include analyzing residents’ health status, goals and diet restrictions, tracking residents’ oral intake and progress and educating residents on a person-centered basis. 

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES: Employee will apply skills to ensure Person Centered Care as exhibited through our Pillars of Excellence:

· Conduct comprehensive nutrition assessment, education, and management to residents admitted to the health care center.

· Provide education on the use of modified diets as it relates to health

promotion and chronic conditions.

· Coordinate, implement, and evaluate nutritional education using National standards and guidelines.

· Evaluate residents for their individual needs and develop plan of care to meet the goals for discharge.

· Develop and deliver educational programs and appropriate teaching tools for various ethnic and cultural groups to support understanding of nutrition.

· Collaborate with fitness professionals and medical staff to focus on aging adults living full active lives.

· Teach a wide range of classes integrating physical and emotional well-being. 

· Provide community-based education and counselling to patients and families, individual or group, in accordance with established educational plans.

· Prepare nutrition related presentations and present them to various audiences.

· Maintain timely and complete documentation of all nutrition management encounters.

· Comply with applicable federal, state and local laws, rules, and regulations; maintain knowledge of and follows all Plymouth Place policies and procedures.

· Maintain regular and punctual attendance at work and meetings.

Requirements

· Bachelor’s degree with course work approved by the Academy of Nutrition and Dietetics

· Completion of Dietetic Internship, current Registered Dietitian, and Registered Dietician License in Illinois

· Sanitation certification

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

  • Ability to work independently and rapidly develop productive and collaborative relationships with other healthcare providers.
  • Knowledge of principles and case management guidelines for rehabilitation and long-term care.
  • Recognize and address the unique culture, language, health literacy and age-appropriate care of diverse patients and caregivers.
  • Experience in Point Click Care (PCC) and MDS.
  • Ability to teach, coach and train competencies. 
  • Charting knowledge for needs of the residents’ nutrition care plan, following the International Dietetics and Nutrition Terminology Manual 
  • Knowledgeable of new dietary standards/IDPH guidelines.
  • Proficient with Microsoft Office Suite or related software.
  • Exceptional communication and customer service skills.
  • Solid organizational skills including attention to detail and multi-tasking skills.
  • Ability to read, write and speak English sufficient to perform job duties and to interact and communicate with residents, families, and employees.
